Patient Details

This is a 38 year old male who is 6’1″ and 180 pounds who presented to me complaining of fullness in his nipple areola area on both sides after having had previous gynecomastia treatment with liposuction alone. His primary concern was his puffy nipples. You can clearly see that he has fullness on both sides of his chest, larger on his right than the left. He is seen here after a gynecomastia surgery involving the removal of 350cc of fat and a small amount of glandular tissue beneath each nipple-areola complex. His only complaint is a small crease at the bottom of his right areola when he raises his arm.He complained that on a scale of one to ten, his gynecomastia was a 10 (meaning it really bothered him). He tells me now that is “all gone”,  “it’s a zero”.We discussed the minor irregularity. We both agreed that it’s quite inconsequential and if that’s the worst price to pay for not having gynecomastia anymore he would take that in a heartbeat. Lesson to learn: Results from gynecomastia treatment aren’t always perfect in the patients mind. Minor contour irregularities are almost inevitable in many cases depending on their anatomy (how much fat, fibrofatty and glandular tissue is present). Many of my gynecomastia patients are quite analytical and this can sometimes get in the way of an otherwise impressive result. In some cases, I don’t even think that these patients even know what “normal” is. You can clearly see in his photos that he no longer has puffy nipples, that his areolas are flatter and smaller and that he no longer has the visible mass behind his nipple-areola areas. Chest hair is also a wonderful tool to take the focus away from the nipple-areola region.
