Patient Details

This was an advanced gynecomastia case. The patient is 58 years old and presented with compromised skin elasticity related to age, significant sagging of the chest, and fullness extending into the underarm (axillary) area. In cases like this, my primary concern is always skin retraction—because the quality of the skin plays a major role in the final result. Simply put, the better the skin, the better the outcome.

We discussed all appropriate options in detail:

  • Leaving the condition untreated
  • Standard gynecomastia treatment without skin removal
  • Gynecomastia surgery with skin excision, which would involve visible scars across the chest and into the underarm region

After understanding the trade-offs, the patient elected to proceed with standard treatment.

If you’re not deeply familiar with what a chest like this typically looks like after surgery, it’s important to say this clearly: this is an impressive result. His skin retracted remarkably well, and the chest contour is excellent—something that is never guaranteed in patients of this age and skin quality.

Postoperative skin management and strict compliance with my gynecomastia aftercare instructions were essential to achieving this outcome. Surgery is only part of the equation—what happens afterward matters just as much.

Today, he’s back to wearing his favorite polo shirts while golfing and cruising on his boat—without thinking twice about his chest.
