As you can clearly appreciate, his nipple-areola complexes became smaller and migrated “up and in” following treatment. This is a phenomenon I routinely observe after properly performed gynecomastia surgery and one of the reasons I am often able to avoid more aggressive skin removal procedures with long scars across the chest and around the areolas.
His skin retraction is quite good, though not perfect, with some minor contour irregularity on one side. Importantly, however, this patient avoided the significant scars commonly associated with skin excision techniques. The end result is natural, masculine, and dramatically improved.
This case also highlights an important concept in gynecomastia surgery: perfection is not always the goal—improvement is. Most advanced gynecomastia patients understand they are starting from a difficult position and are not expecting absolute perfection. What they want is freedom from the appearance of gynecomastia, a more masculine chest, and the confidence that comes with meaningful change. In this patient, the transformation for the better is simply remarkable.
I spend a great deal of time educating patients so they maintain realistic expectations regarding what surgery can and cannot achieve. I often tell patients: “It may not be perfect, but it can be perfectly amazing.” My philosophy has always been to underpromise and overdeliver whenever possible.
