Gynecomastia—the benign enlargement of male breast tissue—often begins subtly: a tender lump beneath the nipple, sensitivity to touch, or a gradual sense of fullness. One of the key indicators that the tissue is active and inflamed is pain. This discomfort is more than just annoying—it’s a clinical tip-off that the tissue is in a growth phase, and this window offers a potential opportunity for early intervention.
One promising, underused treatment in such cases is intralesional corticosteroid injection. By injecting a steroid like triamcinolone acetonide directly into the firm tissue beneath the areola, we can deliver a powerful anti-inflammatory effect exactly where it’s needed. These steroids work by suppressing local inflammation, dampening fibroblast activity, and in some cases, reducing the size and density of the developing gynecomastia tissue.
This treatment is most effective in the early stages, when the glandular tissue is still soft, hormonally active, and not yet fully fibrotic. Patients who act quickly—especially those who notice new pain or swelling—may benefit from this non-surgical, in-office procedure. Some cases require multiple injections spaced a few weeks apart, and results vary depending on the timing and underlying cause (such as hormones, medications, or supplements).
It’s important to understand that once the tissue matures into a dense, rubbery gland, steroid injections are no longer effective. Surgery remains the only option in such cases. But for patients who recognize the warning signs early—especially pain and tenderness—steroid injection may offer a chance to stop the progression or even reduce the tissue before it reaches that stage.
