Gynecomastia Today: It’s Not Just Puberty Anymore

If you’ve spent any time reading about gynecomastia, you’ve probably seen the common explanation: “It’s usually caused by hormonal changes during puberty, and it often goes away on its own.” That may have been true once. But here at the Austin Gynecomastia Center (AGC), where I’ve treated over 6,000 men with this condition, I can tell you that the game has changed.

In fact, the most common cause of gynecomastia I see today isn’t lingering puberty-related tissue—it’s the modern wave of hormone-altering drugs and supplements that many men use every day, often without realizing the risk.

Let me explain.

Puberty Was the Old Story. Today’s Reality Is Different.

Gynecomastia (the development of excess male breast tissue) has traditionally been chalked up to natural hormonal fluctuations during adolescence. Estrogen rises, testosterone lags, breast tissue may grow—and for many teenage boys, it resolves within a few years. End of story.

But that’s not what I see in my practice anymore.

At AGC, the majority of patients I treat are adult men—often in their 20s, 30s, 40s, and even older. Many of them didn’t have gynecomastia as teenagers. What they all seem to have in common is exposure to testosterone, anabolic steroids, SARMs, finasteride, anti-aging treatments, or performance-enhancing supplements.

In short, they’ve taken something—often legal, prescribed, or over-the-counter—that disrupted their hormonal balance. And the result is permanent breast tissue growth.

Anecdotal? Yes. But the Pattern Is Too Strong to Ignore.

Let me be clear: there are no large-scale studies that confirm what I’m seeing. But when you’ve operated on over 6,000 gynecomastia patients—many of whom come from all over the world—you begin to notice patterns.

And the pattern is this:

Today’s gynecomastia epidemic is being fueled not by nature, but by modern hormone use.

We’ve created a perfect storm:

  • Testosterone therapy is widely prescribed, often without proper estrogen management.
  • SARMs are sold online and marketed as safe alternatives to steroids.
  • Hair-loss medications like finasteride alter hormonal pathways.
  • "Natural" supplements often contain hidden hormone-active ingredients.
  • And body-conscious culture pressures men into trying anything to gain an edge—at the gym, in the bedroom, or simply with aging.

Even a mild imbalance—whether it’s increased estrogen activity or a lack of testosterone’s suppressive effect—can stimulate growth of breast tissue. And once it’s there, it doesn’t go away. No cream, no pill, no amount of exercise will reverse it. Only surgery can.

The Psychological Toll Is Real

These aren’t just guys chasing vanity. Many are deeply ashamed, hiding their chest with baggy shirts, avoiding intimacy, skipping the beach, and living with daily anxiety. And the worst part? They’re often blindsided. No one told them this could happen when they started that supplement or filled that TRT prescription.

The mental weight of gynecomastia is real. It’s not about cup size—it’s about control, confidence, and the core of a man’s identity.

So What’s the Takeaway?

If you’re a man with gynecomastia and you’re wondering, “Why me?”, take a closer look at what you've been exposed to.

Ask yourself:

  • Have I ever taken testosterone or anabolic steroids?
  • Do I use SARMs or prohormones from supplement shops or online vendors?
  • Am I on finasteride or other hair-loss drugs?
  • Do I use over-the-counter testosterone boosters or anti-aging products?

If the answer is yes, your gynecomastia may not be a mystery after all.
