Skin Removal Gynecomastia Surgery: A Remarkable Transformation

This is an exciting case that illustrates when skin removal can make a profound difference in gynecomastia treatment. At just two weeks after surgery, this patient is healing beautifully, with 100 percent survival of his free nipple grafts. Although his scars and chest contour will continue to mature over the coming months, his transformation is already more than physical: new confidence, new clothing, a new attitude, and the freedom to feel comfortable in his own skin.

Skin removal gynecomastia surgery differs substantially from my standard approach, which removes gynecomastia tissue while preserving the overlying skin. Often called “double incision gynecomastia surgery,” this operation removes both the breast tissue and the excess skin to create a flatter, more masculine chest.

In this patient’s procedure, the nipple-areola complexes were removed, carefully prepared, and transplanted onto the reshaped chest as free nipple grafts. This allows their size and position to be tailored to the new chest contour. The grafts must then establish a blood supply from the underlying tissue.

Special protective dressings called bolsters hold the grafts securely against the chest during this critical early healing period. These generally remain in place until the postoperative visit at approximately five days, when we remove them, evaluate the grafts, and provide instructions for continued wound care. Patients traveling to Austin should plan to remain through this visit or arrange to return to our office five days after surgery. The bolsters should remain dry and undisturbed until our team removes them.

This procedure involves permanent scars across the lower chest and around the nipple-areola complexes. The incisions may extend toward or beneath the arms when excess skin in those areas also needs treatment. Scars usually soften and fade with time, but they do not disappear. Free nipple grafting can also cause lasting changes in nipple sensation, pigmentation, and projection. These are important trade-offs to understand before surgery.

Skin removal is best suited to selected patients with substantial sagging, significant skin excess, and poor skin elasticity, often following major weight loss. However, a large chest or the appearance of loose skin does not automatically mean skin removal is necessary.

Throughout my career, I have emphasized that the chest skin’s capacity to retract after appropriate gynecomastia treatment should not be underestimated. Many men who initially appear to need skin removal can achieve excellent results with my skin-preserving approach, avoiding the longer scars. Others, like this patient, benefit tremendously from removing skin that cannot adequately retract. The key is choosing the right operation for the individual.

If you have been told that you need skin removal, I invite you to submit a consultation to the Austin Gynecomastia Center. I will personally evaluate your anatomy, skin quality, and goals and provide my experienced recommendation.
