Zone 1 Gynecomastia: Two Years After Complete Tissue Removal

This patient is two years out from Zone 1 gynecomastia surgery. His chest hair makes the change subtler in these photos. Look closely at the area around the nipples. The puffiness is gone, the chest has a smooth, natural contour, and the small scars along the lower edge of the areolas are barely visible.

Chest hair can disguise gynecomastia in a shirtless photo. Put on a fitted T-shirt, though, and the tissue underneath can still push the fabric forward. That’s where many men appreciate the difference most. You can reach for the shirt you want to wear without first checking whether it shows your chest.

The firm mass beneath the nipple is what I call the engine of gynecomastia. I remove it completely, including the tissue directly beneath the nipple and areola. This helps flatten puffy nipples and reduces the chance of recurrence. I use liposuction to blend the surrounding contour and a layered closure to bring normal tissue beneath the areola for support. That allows me to remove the mass without deliberately leaving a cuff of gland behind to fill the space.

I described this approach in my published study of 567 cases, Total Gynecomastia Removal with Layered Closure. At two years, this patient shows what I aim for: a chest that looks natural shirtless, lies flatter under a shirt, and has scars you have to look closely to see.
